Summary: Background talkers and noise negatively influence speech recognition, but far less is known about the effects of acoustic background competition on speech comprehension and memory. This course discusses experiments that indicate that background voices and noise exert a negative influence on spoken narrative comprehension and memory, even when narratives have been presented at positive signal-to-noise ratios (SNRs) that ensured accurate target speech recognition.

Summary: The session describes a case study that investigated the effectiveness of using a Comprehensive Integrated Approach to treat stuttering with a bilingual-bicultural Spanish-English adult who experienced stuttering. The case study queried whether a monolingual English-speaking clinician could effectively guide a bilingual-bicultural Spanish-English speaking adult who stuttered to transfer skills to a second language. The intervention addressed the client's bilingual-bicultural performance on measures of speech fluency, strategy use, reactions to stuttering, problem-solving, and quality of life.

Scheer-Cohen, Heisler, and Moineau outline an approach to holistic admissions that includes a video response to a question, an informal group interview, a live lecture with an assessment, a simulation, content quiz, a writing prompt, and an individual live interview. Summary: These SIG 7 Perspectives articles focus on auditory rehabilitation (AR) for adults with cochlear implants. While the benefits of AR in the population are recognized in the literature, service-delivery models are variable, and there is no gold standard approach to developing and implementing a comprehensive AR program. Glade and colleagues provide an overview of clinical models currently being used for the provision of AR for adults with cochlear implants from nine clinics across the country. The article highlights the importance of interprofessional practice in AR and outlines the roles of professionals included on care teams. There is a discussion about barriers to successful implementation of AR programs, including distance to services, and recommendations for potential solutions, such as teletherapy. In the second article, Mosley describes the process of creating and implementing a comprehensive teleaudiology AR program for older adults who use cochlear implants at the University of South Alabama Speech & Hearing Center.

Summary: The audiogram is a poor representation of a person's underlying otopathology and can therefore be a poor predictor of a person's hearing impairment. For example, persons with audiometric thresholds within normal limits may experience hearing impairment such as difficulty hearing in noise. Cochlear synaptopathy and outer hair cell dysfunction are two otopathologies undetected by the standard audiogram (a.k.a. Hidden Hearing Loss), but outer hair cell function plays a primary role in hearing in noise performance. A third undetected otopathology is cochlear untuning, which occurs secondary to outer hair cell damage. This course discusses each of these otopathologies and presents clinical normative data that can be used to differentially diagnose each otopathology.
