Enhancing Health Literacy, Emphasizing Inclusion, and Empowering Informed Decision-Making in Post-Acute Care (PD102530)
As a result of the COVID-19 pandemic, access to our patients for in-person treatment may be limited and our ability to develop a relationship with patients/caregivers may have taken a toll due to facility protocols to protect against the spread of infection. As health care providers, we must reflect on our abilities to communicate with patients and their families, ensuring understanding of their deficits and treatment options even as we continue through a pandemic. This course provides the clinician with a review of holistic assessments and health literacy approaches that will empower the patient to make informed decisions.

Presenter Information
Danielle Connor, MS, CCC-SLP (Presenting Author), is a Clinical Services and Education Specialist for Reliant Rehabilitation. As an ASHA-certified speech-language pathologist, she has experience in a variety of settings, including acute care and long-term care, with a passion for post-CVA care. In her current role, she assists in developing continuing education, policies and procedures, and new educational tools to support field therapists. She was a speaker at the 2020 OSHA and the 2020/2021 TSHA conferences.

Annie Govea, MS, CCC-SLP (Nonpresenting Author), is a member of the Clinical Services and Education Specialists for Reliant Rehabilitation and has experience working in various settings, including schools, SNFs, and home health, and performing mobile MBSSs. Her experiences as director of rehabilitation in SNFs renewed her commitment to improving rehab care for adults. Now, as a member of the clinical team she is able to provide trainings and collaborate on education with a team focused on enriching the rehabilitative care provided to patients.
