Summary: This course includes three recorded sessions from the 2016 online conference "Maximizing Functional Outcomes for Patients With Dementia." These sessions focus on working with all the stakeholders involved in treating patients with dementia – the individual, family members and other caregivers, and all the health care professionals involved in the person’s care. Sessions discuss caregiver counseling and support, helping patients with hearing loss, and a specific model for interdisciplinary care. The conference included a total of 13 sessions, with the broad goal of describing a range of evidence-based clinical care techniques to get to the heart of patient-centered dementia care.

Summary: What is the audiologist's role in providing care for children and adolescents who have experienced concussion/mild traumatic brain injury (mTBI)? This on demand webinar discusses advanced testing of the central auditory and vestibular pathways for young people with concussion/mTBI as well as those with post-concussion syndrome. Whether you have equipment and are ready to start an mTBI protocol in your clinic or you want to learn more about the nature of the symptoms of mTBI, this course provides the tools to help you assist your pediatric patients.

Summary: This session reviews what it means for a measure to be objective, subjective, and/or valid and applies these concepts to fiberoptic endoscopic evaluation of swallowing (FEES). The presenters also discuss other, equally important, criteria to consider when evaluating a patient for dysphagia: accuracy, meaningfulness, and relevancy.

Summary: In “Coupling Hearing Health With Community-Based Group Therapy for Cognitive Health in Low-Income African American Elders,” Postman et al. describe a community-based group intervention to address disparities experienced by African American elders in the early stages of cognitive–communicative decline. The intervention included partnerships with community health centers, culturally informed activities, and ongoing input from staff and participants. The authors describe the advantages of this community-engaged approach, as well as the benefits of joining hearing and cognition for minimizing access barriers. In “Public Health Frameworks in Audiology Education: Rationale and Model for Implementation,” Warren and Levy review how public health education can advance the field of audiology, particularly through coursework and dual degree programs. The authors also describe two frameworks for public health training in an audiology academic setting and identify the competencies that overlap in audiology and public health, helping to illustrate the relevance of public health education in addressing objectives in hearing health care.

Summary: Recent studies from the presenters’ laboratory demonstrate a relationship between electrode-to-modiolus distance and channel independence. Specifically, children and adults who use cochlear implants (CIs) (precurved electrodes) demonstrate performance gains up to 12 and 16 channels. The presenters’ working theory is that greater channel independence affords better spectrotemporal resolution. This presentation describes the relationship between spectrotemporal processing and CI outcomes for adult and pediatric CI users. This course was presented and recorded at the 2019 ASHA Convention. This session was developed by, and presenters invited by, Hearing, Tinnitus, and Vestibular Science.

Summary: These SIG 7 Perspectives articles focus on auditory rehabilitation (AR) for adults with cochlear implants. While the benefits of AR in the population are recognized in the literature, service-delivery models are variable, and there is no gold standard approach to developing and implementing a comprehensive AR program. Glade and colleagues provide an overview of clinical models currently being used for the provision of AR for adults with cochlear implants from nine clinics across the country. The article highlights the importance of interprofessional practice in AR and outlines the roles of professionals included on care teams. There is a discussion about barriers to successful implementation of AR programs, including distance to services, and recommendations for potential solutions, such as teletherapy. In the second article, Mosley describes the process of creating and implementing a comprehensive teleaudiology AR program for older adults who use cochlear implants at the University of South Alabama Speech & Hearing Center.

Summary: Impairments in laryngeal vestibule closure (LVC) are a major cause of aspiration during swallowing. Accurately identifying LVC impairment is a priority in dysphagia management as aspiration can occur if LVC is absent or delayed, or duration is reduced. However, this mechanism is often overlooked and underreported in the evaluation of swallowing and in swallowing research. This session discusses methods for incorporating LVC as a primary outcome measure in dysphagia diagnosis and management to increase diagnostic accuracy and optimize dysphagia management.

Summary: Streamed audio has grown from a technological novelty into a distinct listening environment for hearing aid users. This presents a unique listening environment that can prove difficult to verify, as streamed audio is not an external stimulus and cannot be verified using conventional methods. This session describes a new and accessible verification method aimed at the streamed audio environment, ensuring that hearing aid users receive optimal benefit in their digital soundscape.

Summary: Older adults must make important decisions about their end-of-life care, and those with hearing and other communication disorders need special accommodations to participate in these important decisions to the fullest extent possible. This session discusses the communication needs of patients in the final stages of life, including the use of various hearing technologies, such as hearing aids, personal sound amplifiers, and pocket talkers. The presenters discuss tools to facilitate conversations among patients, family members, and caregivers.

Summary: This quartet of SIG 13 articles provides information regarding managing and treating dysphagia in the adult population. Caileen Harvey, Rachel Flemming, Julia Davis, and Victoria Reynolds investigate International Dysphagia Diet Standardisation Initiative implementation issues by surveying health care professionals in health care facilities in rural Upstate New York. Ankita M. Bhutada, William A. Broughton, Brenda L. Beverly, Dahye Choi, Sandip Barui, and Kendrea L. (Focht) Garand aim to identify the prevalence of dysphagia and reflux reported symptoms in patients with obstructive sleep apnea syndrome and determine associations between symptoms and demographic and clinical variables. Stevie Marvin summarizes published research on screening, evaluating, and treating post-extubation dysphagia in the intensive care unit. Rebekah Guastella, Stefania Oppedisano, Luis F. Riquelme, and Ashwini M. Namasivayam-MacDonald study bolus location at swallow onset, stage transition, pharyngeal transition duration, pharyngeal response duration, and pharyngeal phase duration between cued and uncued swallowing conditions in patients with dementia.
